Job Title: Registered Nurse Job Specialty: Home Health Job Duration: 13 Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: - Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty - First-timers not accepted; must have recent specialty experience License: Must have license in hand Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase (HCHB) EMR, used within the last 6 months, or completion of a retraining course during compliance - Proficiency in OASIS Job Description: - Provide, coordinate, and direct the provision of home nursing care according to physicians' orders and agency policies through the competent application of the nursing process - Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses - Conduct home visits and attend periodic office meetings - Manage an average of 5-8 patients per day - Achieve weekly productivity expectations of 30 points, with point values assigned for each visit and activity - Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. - Willingness to travel and cover a territory radius of up to 75 miles from the home clinic - Flexibility to work in a standard 5x8 schedule primarily from Monday to Friday, with the possibility of a weekend rotation 1-2 times per month - On-call duties required 1-2 times per month - Responsible for completing all documentation within 24 hours of patient appointments - Must have reliable transportation, a current driver's license, and agency-required liability insurance - Mileage reimbursed at IRS standard rate; travel compensated from patient home to patient home Please ensure that you can meet the travel, documentation, and patient care expectations, as well as the scheduling requirements.
